2. Call 423-246-5111 to ask any questions that you may have and set your fee.  You must then fill out and mail these required forms to:  225 Broad Street, Suite 2, Kingsport, TN. 37660.
3. After receiving these your forms, we will contact you to schedule your session and collect your set  fee to hold your appointment time.

*Some special exclusions may apply to Doxy or Facetime availability. It is dependent on the client's counseling issue. A request can be made but the counselor would make the final decision regarding these appointments.
